Registering for a class is as easy as 1-2-3!

We offer 3 easy methods for registering your puppy or adult dog for a training class.

Fax, e-mail or phone!  Choose the one that suits you best!

Puppies With Purpose is on Wednesdays in Skokie.  Agility Dog Training classes are on Thursday evenings in Chicago.

Be sure to select your class type, location and start date before beginning your registration process! 

For 'round the clock Express Registration via fax, follow these 3 simple steps:

1. Make a note of the new class start date (shown on the right of this screen) and

class time in which you are interested (i.e. 6:00 PM vs 7:00 PM (if offered) for Agility)

2. Print and complete the registration form found below.

3.  Fax to 773-384-1310 for AGILITY

    Fax to 847-677-6448 for Puppies With Purpose 

For 'round the clock Express Registration via e-mail, follow these 3 simple steps

1. Make a note of the new class start date (shown on the right of this screen) and

class time in which you are interested (i.e 6:00 PM vs 7:00 PM (if offered) for Thursday Night Agility)

2. Download  and complete the registration form found below.

 For registration via your phone, follow these 3 simple steps:

1. Make a note of the new class start date (shown on the right of this screen) and

class time in which you are interested (6:00 PM vs 7:00 PM (if offered) for Agility classes on Thursday night.)

2. We two different registration hotlines to make you're registration experience easier!

All Fours Agility Registration: 773-384-5959 (Between 8:00 AM - 6:00 PM M-F)  

Specify the All Fours Agility Class Registration with Dee on Thursday Evenings

and include the start date and time that you've chosen for Agility.

3. Puppies with Purpose Registration: 847-677-0696 (Between 9:00 AM - 4:00 PM M-F)

Specify the Puppies With Purpose start date.

Start dates and times for Agility and Puppies With Purpose can be found on the right side of this screen.

Below you'll find your registration form.  It can be used for both our group Agility Classes in Chicago, IL. on Thursday evenings and our Puppies With Purpose training classes on Wednesdays in Skokie, Illinois.
